Home>Access & Inclusion Plan - Pyrenees Shire
| The Disability Discrimination Act 1992 (DDA) prohibits discrimination on the basis of disability. The DDA allows for the development of an Access and Inclusion Plan (AIP) to assist Council to change any practices which may be discriminatory. As part of the development of the AIP for the Pyrenees Shire we would like you to assist us in identifying areas in need of change.
Please answer the questions in the survey below by ticking the appropriate box/es and/or adding your comments where indicated.
This information is confidential and only for use in the development of the AIP. However, if you would like to be directly informed about the progress of the plan and would like a draft of the Plan mailed to you for comments, or be part of a future advisory group, please enter your contact details at the end of the form.
Please note: If you need any assistance filling out this form or if you require alternate formats, please contact Norman Prueter on 5349 1183 or email: norman.prueter@pyrenees.vic.gov.au. Please complete the form and return to the Beaufort Community Resource Centre in Neil Street or if one has been provided to you, in the self addressed envelope, by September 18th 2009.
